Frequently Asked Questions

How long does the evaluation take?

Most evaluations last 30–60 minutes. You will receive more specific timing information when your appointment is scheduled.

What should I bring to the appointment?

Please bring the following items, if available:

  • A list of current medications

  • Recent and relevant medical, developmental, or psychological reports

  • Recent evaluations related to your disability claim

  • Individualized Education Plans (IEPs) or Individualized Family Service Plans (IFSPs)

  • Any hearing or vision screening results

  • A photo ID (required for entrance at the Chicago office)

Can I stay with my child during the evaluation?

Yes. Parents and caregivers are welcome to remain in the room. However, some children test more effectively when evaluated independently. Your evaluator will make recommendations based on your child’s needs.

Do you provide diagnoses?

Yes. When appropriate and supported by results, diagnostic impressions are included. All reports are written in clear, descriptive language that is helpful for the disability determination process.


Office Policies

Only water is permitted inside the office (no food or other beverages).

To maintain a quiet and respectful environment, especially for individuals who may be medically fragile, please limit to one support person per appointment.

Please make childcare arrangements in advance for any children not being evaluated.


If you cannot attend your appointment

What should I do if I cannot make my scheduled appointment?

Please notify your adjudicator (case worker) as soon as possible. All scheduling and rescheduling is handled directly through Disability Determination Services (DDS), not through our office.

To request a new appointment, contact your adjudicator by calling DDS in Springfield at 1-800-225-3607. Your adjudicator’s name and extension are listed on the letter you received from the State.

We also ask that you inform our office at 312-219-5901 if you will not be attending. Please note: our office cannot arrange a new appointment unless it is authorized by DDS.

Who should I contact with questions about my claim?

Questions about your disability claim or the overall application process must be directed to your DDS adjudicator.
